PRIVACY POLICY AT CHUNKYMONKEY.COM

If you're under 13, a parent or adult guardian must print out and sign this form saying that it's okay for you to join the club.  Include the form along with a short essay (at least 25 words) on why you like Chunky Monkey OR a drawing that you've done of Chunky Monkey (which might get posted online!) in a self-addressed, stamped envelope to: Chunky Monkey Fan Club, P. O. Box 64, Absecon, NJ  08201.  Be sure to include the self-addressed, stamped envelope or we won't be able to send you back your membership card.
